The Helping Others Gain Apartment Re-Entry (H.O.G.A.R.) Program, funded by the Illinois Department of Human Services, Division of Mental Health, helps transition individuals with a mental illness residing in Illinois nursing homes directly to the community through the provision of intensive cash management services.

The “Money Follows the Person” state initiative seeks to address the behavioral needs of individuals experiencing a chronic mental illness inadequately housed in nursing home settings, who can greatly benefit from intensive, community-based services that support improved independent functioning, self-sufficiency, and positive long-term health outcomes.
