  The Intensive Case Management/Community Support Team (ICM/CST) Services, funded by IDHS/DMH, provides structured services to persons diagnosed with a severe mental illness. The program aims to reduce the frequency of hospitalizations and targets those individuals possibly isolated from supportive services. A low staff to participant ratio (18:1) allows for more individualized attention and follow-up activities. Staff renders services primarily in community settings and often in the participant’s home.

Services include Clinical assessment; Treatment planning; Psychiatric services; Medication monitoring and training; Individual and group counseling; Peer support groups; Intensive outreach; Advocacy, linkage and external consultations with other providers; Active community integration through case management and social rehabilitation; and 24-hour emergency services.
