The Linkage Case Management Services, funded by the Illinois Department of Human Services, Division of Mental Health, support individuals discharged from in-patient psychiatric care or triaged to the Center from an emergency room in lieu of hospital admittance. These services address the needs of persons with acute psychiatric conditions to optimize program participant functioning in a community setting.

Program staff members attempt to stabilize individuals using crisis response techniques which help reduce recidivism rates, avoid costly in-patient care and/or reduce lengthy hospital stays. Services are delivered at an intensity level that supports the individual in the least restrictive, most clinically appropriate setting.
