Job Description

Pilsen Wellness Center, Inc. is a community-based not-for-profit agency that provides culturally sensitive services to a multicultural population with limited financial resources. The organizations mission has remained providing quality human services to economically disadvantaged children and families. The agency is comprised of five divisions – Mental Health, Substance Abuse, Youth Services, Early Intervention, and Education which are currently supported through 15 different locations.

The Project for Assistance in Transition from Homelessness (PATH) Program provides mental health community-based outreach and case management services to the homeless populations that have mental illness. PATH goals seek to establish community relations which allow staff to do outreach and intake for persons who are homeless and/or at risk of becoming homeless.

We are seeking a highly motivated Mental Health Professional to join our PATH Program located at 2015 West Cermak Road Chicago, Illinois 60608.

Work Schedule available is Monday-Friday 9am- 5pm.

Responsibilities and Duties

This individual participates in the development and implementation of individual service plans in order to increase a participant’s independence skills in daily living, economic self-sufficiency, and community integration which may include an array of services and interventions designed to provide treatment, habilitation, training and other community integration services.

Qualifications and Skills

A Bachelor’s Degree in Counseling; Psychology; Social Work and/or 3-5 years of mental health experience or related field required. Driver’s License required.
