Pilsen Wellness Center, Inc. is a community-based not-for-profit agency that provides culturally sensitive services to a multicultural population with limited financial resources. The organizations mission has remained providing quality human services to economically disadvantaged children and families. The agency is comprised of five divisions – Mental Health, Substance Abuse, Youth Services, Early Intervention, and Education which are currently supported through 15 different locations.

We are seeking highly motivated part time Case Manager to join our team in the 510 FEP Program located at 2259 S Damen Ave, Chicago IL 60608

The work schedule available is: Tuesday-Friday

Responsibilities and Duties

Under the supervision of the Program Director and 510 FEP Program Supervisor, this individual is responsible for Case Management Services including assisting participants with accessing the community resources, coordination, linkage services, and developing the skills necessary to increase independence and functioning in the community.

Qualifications and Skills: High School Diploma or GED

Mathematical Skills: Must be able to understand basics mathematical processes in order to prepare numerical reports and spreadsheets.

Language Skills: Bilingual (English/Spanish) REQUIRED. Must possess good verbal/written communications skills in both English and Spanish to converse and correspond with participants, staff, family members, and others as needed.
