Case Management Services

The Intensive Case Management/Community Support Team (ICM/CST) Services, funded by the Illinois Department of Human Services, Division of Mental Health, provides structured service to persons diagnosed with a severe mental illness. The program aims to reduce the frequency of hospitalizations and targets those individuals possibly isolated from supportive services. A low staff to participant ratio allows for more individualized attention and follow-up activities. Staff render services primarily in community settings and often in the participant’s home.

Services include: Clinical assessment; Treatment planning; Psychiatric services; Medication monitoring and training; Individual and group counseling; Peer support groups; Intensive outreach; Advocacy, linkage, and external consultations with other providers; Active community integration through case management and social rehabilitation; and 24-hour emergency services.

One of the Community Support Teams also receives funding from the Illinois Department of Human Services, Division of Mental Health to offer services to Colbert Class Members.
